slopvomit answer in full after explaining what it was:

__systemd_osc_context_precmdline and __systemd_osc_context_ps0. These are systemd's newer shell integration hooks — they emit OSC 3008 escape sequences before and after every command to enable terminal tracking features (command status reporting, shell context, etc.).
The login shell has:

PROMPT_COMMAND with __systemd_osc_context_precmdline injected
PS0='$(__systemd_osc_context_ps0)

The child bash has neither. That's the entire diff that matters.
These functions blast \033]3008;...\033\\ escape sequences into the terminal output stream around every command. If the client-side terminal emulator doesn't understand OSC 3008 (and most don't yet — it's a very new systemd thing), the terminal enters a state where it's trying to parse an unrecognized OSC sequence and silently consumes everything until it sees a proper string terminator. Sometimes it finds one, sometimes it doesn't — hence the ~50% success rate and the seemingly random output dropping.
This shipped in a recent systemd update on Arch, probably sitting in /etc/profile.d/ as something like systemd-prompt.sh or similar.
Fix: either remove/rename that profile.d script, or just unset the hooks in their .bashrc:

This is a known growing pain with systemd's terminal integration. Their local terminal handles it fine because it's probably a newer version of whatever emulator they're using, but the SSH client's terminal isn't parsing those sequences correctly.
