you'd just move the mesh in edit mode.
the origins/bones between visual/physics smd are mismatched
By edit mode you mean in blender right?

your mesh of the model itself and the collision mesh must be tied to the same bone, otherwise you get what is in the screenshot
one last thing, this happens when I equip the physgun / grav gun
how do I fix this? does this have to do with the bones?
