it is already in the game and it is as awesome as you say. i don't think i could enjoy a non permadeath game. I think it could be improved in two ways tho:

• surrendering to the police so you get arrested instead of executed
• non lethal damage (fists, maybe a contundent object, falls...) that puts you into the hospital bed instead of getting killed
