No higher then it is already. NPCs smoking will already give it the tobacco use tag. The bloody murders already will set it to teen and cigars will not make it any higher. Sexual content or large amounts of gore are what will make it much higher.

We can get ****face drunk on a variety of alcohol already. That plus bloody murder and NPCs walking around in their underwear already set a decently mature tone. So yeah smoking would fit right in.

I'd love to get the full Columbo experience, but it does make me wonder what effects smoking would have on the player – given that the devs have put a ton of attention towards buffs/debuffs, even down to an addiction mechanic via the Sugar Daddy sync disc.

It could give the same relaxed buff sleeping does without removing tiredness. Tobacco is a depressant with a calming effect.

It could give a small warmth bonus. But honestly, even if it was all negative it would still be cool. I still get drunk and throw the bottles at enforcers for no benefit after all

In my experience smoking has never once benefitted me physically. It just makes normally easy stuff harder.
