yeah I'd agree with Ceaușescu and Honecker
does Khrushchev count?
I dont think Honecker was nearly as bad as Ceaușescu or Hoxha. The former one was brutal but at least those actions were not done with no reason behind them. Meanwhile the later two especially in their later years acted very much on the vim or without rationale.
at least ceausescu had the decency to get overthrown and die

Anyway, I’d note that Ceasescu only ruled for ~24 years. Hoxha ruled for 41.
His government’s rule also did not end with him, as the ACP remained in power for 6 years after his death.
In Romania the successors to the communist party are essentially powerless. In Albania they have been ruling since 2013 and are one of the two main parties
So Hoxha could be worse because his grip on the country lasted for many, many decades
