There were a couple of events you missed, so here's a summary:
Lotus is someone who copied the role of Margulis back near the end of the Old War. Margulis is the one who rescued the Tenno and was our mother figure but also she was Ballas' lover.
Ballas killed Margulis because she said bad words in front of Orokin, that was just the law. Then Ballas got angry and told all the secrets about Tenno to Sentients. Then Ballas and Sentients planned to turn Natah into fake Margulis and kill them from inside.
Somehow instead of fake evil Margulis, she turned into Lotus, and she took up the vacancy of "space mom."
Now, Ballas took back lotus and she kinda turned back into Natah, but as seen in the Prologue cutscenes, something is wrong. In the Orphix Venom event, Lotus is shown to be still alive inside Natah. Lotus sends us a secret message which when solved says "I am dying.
How did ballas became narmer king?
He was on a leash, crawling before erra and kidnapped lotus, and next quest - boom, he's a king now
He was faking being a slave
Being ripped almost in half with his transformation into sentient didn't look like acting
Because that's not part of his acting
He was stabbed in the Sacrifice. He would've died without the transformation
The leash is the acting, acting done in front of Natah.
So right after Erra gave up on convincing Natah and just threw her into the furnace, Erra dropped the act (and the leash) and Ballas stood with him as equals.

So if they went to Duviri, and Ballas didn’t know anything about it, how did he know she came back? Because as Hunhow said, he had his forces searching for her remains.
