Sounds reasonable would add an element to the “survival” aspect.
If, for example, they are presented as mutants or creatures of the zone, then they should have a certain essence, and they will only cause mild anxiety. Alternatively, they may react to the player in some way, such as: I shot at them, and they either disappeared or started chasing me to kill me.
Or if you approach the shadows, they will disappear while laughing (for example).

And your suggestion to appear at sunrise is also a good idea, as it would serve as a greeting for the radius.
However, when I first played the pirate radius and then purchased it, I realized that I was afraid of the nighttime sounds.
It might not be audible, but as soon as evening (night) comes, the volume of the sounds becomes louder in general (I thought there might be some scary shadows, but there weren't). So yes, I think it would be an ideal addition for the night.

A more practical implementation of mental instability might be a reduction of accuracy or introducing a wobble to holding guns. Maybe have some objects like chairs or tables dissappear after first view or appear only after leaving the room and coming back.
Possibly some paths open or close based on your level of sanity. Or an increase in certain spawns.
It's very Cthulu-esque but certainly plays into the weirdness of the Radius.
