This is a small gripe I've had with the game since ITR1. You're on a large expedition in the radius, you're out of syringes, and your last firefight brings you down to low health. You know travelling back to base will be safe, since you already cleared all the enemies, but you know it will be a long journey and you will have the bloody overlay and the sound of your heartbeat playing all the way through.
I would be very happy if the heartbeat noise went away after 10 - 30 seconds of not taking damage. However, this could be a niche and thematic use for the cigarettes. Having a cigarette after a firefight to "calm down" and remove the hurt indicators would be quite immersive. It would give you a reason to carry a pack with you to the Radius, while still remaining a very niche use case, so that people that don't want to engage with smoking aren't missing out on a feature.
