If you're serious, there's a lot of material on the net ready for you to Google and read.
You won't find much help here though. People wanting drones to "carry medicine" 15km away using an encryptable control link tend to be in this for one very particular reason, and we don't do that here.

You need to do much more reading up and research before you're anything like ready for what you describe.
What you are asking for straight up sounds like a weaponised drone, especially with how vague and elusive you're being with the exact payload and mission requirements.
Because of this, I will not help you.

I live in Brazil, here the laws don't work.
look for the tragedy of "Rio Grande do Sul" the private sector that saved the state and the cities, it wasn't politicians or the army, if there was a company that did this work things would be different
