Continuing the topic here!

What we've found:
Objects, enemy and player are all set up correctly.
ID Offsets are seemingly correct too.
MapImage file is repacked correctly with a bonfire picture.
EMEVD IDs correspond to the BonfireWarpParam IDs.
BonfireWarpParams seem correct.

What works:
Bonfire can be lit and used as normal.

Issue:
Bonfire does not show up on the Warp menu.

I have taken a look into the Ids and thought it only searches the specific range for the ListId

here is the vordt bonfire for example, there is a spawn point on top of the player object

the id for the spawnpoint would be +2000 offset from the id of the bonfire "enemy"

The one I copied it from was a 2000 offset from the "Enemy"

I made a new bonfire in irithyll recently with the exact some steps and it works so I have no idea what could cause this

I can do it all over again. If you don't mind giving me a simple step by step I can go ahead and check all the boxes to troubleshoot

I pretty much followed the same step as you
duplicated bonfire object, bonfire "enemy", player object and spawnpoint from a nearby bonfire
changed the collision name accordingly to the new location
gave each of them new unused entityID with the same offset as you did register the bonfire in emevd with the id of the bonfire object, and a new location event id (same one that would be used in param)
duplicate an existing row in bonfirewarpparam then changed the necessarily id (I havent even changed the name and pictureId yet)
dupplicate an existing bonfire ESD to create a new identical one for the new bonfire and put it in the bonfire "enemy"'s talkID (not sure if this one is even necessary
