They can run

people use painkillers before fights/start of raid to suppress pain effects / reduce aimpunch when hit. That way if they get hit they can keep going/fighting without needing to stop and heal. Huge advantage that will save you many times.

because flea "tax" screws you over when you sell it for rubles, so people sell/trade them for items of value

using it or not is an ancient topic of many questions you have to ask yourself

do you play the map often? it is epic loot room, i.e. will it be contested? Would you contest or want to avoid the fight?
