Bullet most likely fragmented. But from what I understand in the After Raid Treatment, you got shot once, the bullet fragmented, and the fragment killed you. So, you received two ticks of damage, but it always displays which sourced killed you last (not necessarily got shot 3 times). First one is the LPS that does 81 damage, then the LPS fragment killed you, hence why it displayed LPS.

theres a bunch of bugs with it for example i know low flesh ammo didnt used to fragment at all even though it had all the stats and i believe it has something to do with armor where if you hit an unarmored part it wont fragment im sure you have noticed it with bosses because they are absolutely cracked and will hit chest shots non stop

Might be wrong, but I don't think fragmentation ever had any major issues. Only issues you mention is bosses being cracked, which was an old issue where they would shoot at insanely fast rate of fire / double shots
