I used to use ifaks since they didn't heal heavy bleeds, that way I could press 4 to heal then 6 if I noticed I had a bleed, but idk if I should stick to that or move to salewas
they also take less space and cost less per hp healed

MDR is you wish, M855A1 is good budget round. The backpack is up to you, but something that doesn't stick out a sorethumb, like the pilgrim. I personally use trizips as they are really cheap through the barter. Oh and, I didn't notice, but maybe also a different grip, something that gives better ergo, if you plan to keep with the RPK.
