I'd wager 99.9% of the people that are cheating AND pvping get banned pretty quickly whereas I'd be more worried about the cheaters that just use it to farm PvE content.

I think there's a lot more closet pvp cheaters getting by than you'd expect. I've met a few in raid that I was able to talk to and they're still not banned
