Well if it's an external pen test that you are asking, firewall rules would still be active, pen testers are there to see if there's a way they can bypass that security, and then provide how to remediate it.

Well they wouldn't need to mask their IP. Red teamers aren't usually operating as black hats. There would be rules of engagement set in place and they would function on their own subnet, unless it called for varying IP attempts. Now if your asking about a red teaming vs blue team engagement where blue team is attempting to block red by IP filtering, there are a few ways to go about mitigating that.
