it can also be hashes depending but it doesn't have to be because otherwise different types of the same malware can be missed

the file hash is better than nothing but some will actually look at the file data and identify series of instructions, strings etc. that have been used in the past
examples could be hashes of domains, ransomware code etc.

In game modification, signatures refers to known sections of code that can be used as a reference rather than offsets which may change so I'd imagine most advanced antivirus solutions would employ that sort of algorithm rather than checking the file hash which could change if a single bit is different
