Not sure if its a bug or not. On the last mission of campaign 4 and even the free mission that starts at mcc. The pressures on for the suit and cabin slowly drop over time.
At first I thought it was an artifact of time acceleration, but the pressures don't go back up. It also doesn't trigger any alarms, even when they go to 0.

I've tried a whole bunch of different settings trying to increase the pressures. The only thing that works is the repress valve above the center seat. And then cabin pressures start dropping again.

Also, the O2 flow level looks like it's at minimum. And I can't get that needle to raise no matter what I do

For the O2 flow use the dial on the CMR side panel (I think it's 8?) with a dial for 02 flow rate or something like that. It's set to min at first when you get to orbit. As for the pressures, make sure all hatches are closed, and that you aren't timewarping above 500x.

I had that set all the ccw. Max flow and it didn't change anything

All hatches closed. Max timewarp of 500x, but it still happens at 250x.
