IMO this should be given high priority as it can really ruin the day of someone who is unaware
just for completion how did you have your SPS set up? there are other things that can keep the shutdown signal from doing anything with an improper comfiguration
I don't recall exactly but it's according to the SPS burn helper checklist (which I need to update at some point)

whats on the checklist and how you physically configured could be different, checklists hold assumptions
I should note that this is a recurring thing
Also the log should say what switches I flipped during flight
try cutting it with the dv thrust switches next time, if it still runs then definitely a bug

there are some burns in early missions where they have you do manual cutoff with the dv switches, I would try that just to solve the problem
flick one off 10 seconds before cutoff and the other off as close as you can

The best practice for burns is to set the EMS up as a backup burn monitor

though in the actual spacecraft you have to think about tailoff thrust and all of that crap
Yeah but that's more a NASSP area of expertise
And iirc tail off can be trimmed with RCS if necessary
