they had to all be players cause NPCs either would "move" out of the way if you get too close, couldn't equip all types of weapons, at times wouldn't allow the kiss swep to work on them.

Go to the texture paint tab, select the armature then the head enter weight paint mode, select the head bone, and just paint the entire face red

Weight paint is what defines the influence a bone has over a mesh, the head isn't following probably because its weight paint is missing and so the head bone has no influence over the head
