There is just a lot of times people would watch/read a small part of something and then they think they have a good understanding of the story so they use the little that they know and work their own image into it

FMA live action wasn't that good as well but it was kind of close to the original material

I would say a mix of the writers and directors, when doing adaptations like this it is very rare that you can present the material following exactly as original, parts need to be changed and they need to decide what needs to be showed and what left out and how to fill the gaps staying in the same world.
