by PC, I mean just the box with the core hardware, like CPU and RAM
if it's smaller, it's probably one of those that are designed to lay on the side, with a monitor on top.
(Anyway, that's less important overall)

Clean the PC for dust, to reduce heat generation (but do not do this alone if you haven't done it before)

Get more RAM (might not be sold for that PC anymore)

Try using a USB Memory stick with Windows ReadyBoost to increase your RAM and potentially improve performance.
Software

Remove all unnecessary software and disable background services.

Tweak Windows Theme settings to reduce overall GUI strain.

Regarding hardware upgrades; old hardware is usually very cheap. The PC isn't compatible with the new stuff.
But yours is so old, I'm not sure it can be found anymore.

If you have money, there are Cloud Computing solutions. Maybe with Free Trials. But I haven't tried any of that.

Ok. It's recommended to reapply as often as every 3 or 6 months. I can't recall. Not many do that anyway.
But 1 year or more, means a reapply will probably help.

Lastly, I don't know what types of optimizations you've made.
There are many tutorials online which can probably help to some degree.
But there's only so much that can be done, when using such old hardware, to make 3D games with terrains.
ReadyBoost might be your cheapest, best option.
The only other thing I can think about, is the extreme measure of using Windows 7's server version (Windows 2008 Server R2) modified for desktop (by using the Win2008Workstation community OS tweaking application).
That was the only thing that made my old rig run League of Legends smoothly.

PS: In Unity, separate objects to multiple scenes. You can load multiple scenes at a time, or just individually. Then it doesn't have to be laggy all the time.
