Act three isn’t a dream I think act 3 is the one where he faces his fears and goes back to the neighbors house and the painting of eyes are his constant fear of being watched but the way he trembles about when he sees the shadow figure in the Neighbors house ruins is because of his ptsd and anxiety

but i also think that the shadow is the guest
because as you know the guest is the alter ego of the neighbor
basically all of his evil and insanity and sorrow
and if the shadow is the guest, it would make sense because he's locking away the evil inside of him
and he's trapped by the maniefestation of the guilt for all the evil things he's done
