Generator Wrench door soft lock

So if you get the wrench, then die, then load from checkpoint you get this soft lock.

there is no promt to remove the wrench btw

I couldn't reproduce this.
Couple of questions:

• Did you ever reload during your playthrough before dying here?
• Did you use the medstation that's behind this door?

Yes, I loaded at the start of the Gen room to continue playing from the day before
No.
I loaded, did the puzzles before with the battery, made it there, got the wrench and made it past, saw the fire covering the door past the wrench door, realized the fire extinguisher was in the Gen room, managed to open the door behind the fire and wondered if I can run through it and live because I was at almost full HP, died immediately, loaded from last checkpoint and had wrench but was back in Gen room, "I'll grab the fire extinguisher before i go over there this time", went back and saw that with no prompt to remove wrench

That’s super strange. Thank you. Did this auto save you reloaded put you in the hallway with the stuck door?

No the save I reloaded put me back in the Gen room

However, I will admit I got confused on the wrench door thing for a while so I was in and out of that room after getting the wrench
