From what you described the tool seems to be deleted as soon you equip it but still show up in your inventory.

the problem is not in the code you provided, unless you share the exact chunk of code that handles the creation of slots for the hotbar, I can't really know what's the issue

there is no image id for the tool
just the text of the tool that shows up in one of the hotbar slots

it seems to be working perfectly fine, and your problem seems to be the "tool model" that doesn't show up when you equip it
it might not be related to the code at all, but the tool, did you weld all the parts together and renamed at least one of them to 'handle'?
