tl;dr quests r ur best friend- every multiple of 4 chapter completions (ex. 1-4, 1-8, etc) gives u diamonds, every multiple of 10 levels in any faction towers gives u diamonds usually, the events usually tell u what u can receive

if ur in a guild, doing the boss challenge mabob can give u diamonds. there's a lot of ways to receive them

it depends on what achievement it is..? cuz typically each boss battle can give u 250 gems, but completing a whole chapter in itself can also give u diamonds (500..?), but for campaign, i just say it varies because depending on where ur at, they also give u other stuff like more heroes or items, etc
