You should receive an automated 'We have received your ticket' email the moment you send a ticket in. Check your spam and trash folders for it if you need to. If you didn't get it, this is a sign that your email you entered is wrong. It doesn't have to be the email associated with your account, just the one you'd prefer to be contacted through.

I didnt get it ( im pretty sure I put it in wrong) but how would I know which day I got banned and what time it happened?

For bans, the exact day and timing isn't needed. The day/time you noticed it is good enough.

Go with the one you got banned under, and in the comments where it asks for your side of things, explain the name change there
