Multikill is a w3 mechanic that based on your total tier of mob you get additional multikill per tier. Tier is determined by how much more damage you do than necessary to kill. Multikill applies exclusively on the first item in the drop table (veiwed as the first item on the card) and acts as a multiplier for how many drops one trigger gives. For example if i farmed a mob with 1/2 chance of dropping a single drop. multikill of 2x would mean i now have a 50% of dropping 2 drops
