Skulls refers to how many difficulties of the boss you kill. So a 5 Skull run will do the most damage possible
After you did that run you can do a daily Misc + Trophy run. Trophies are the things you buy stuff with -> Gold Stopwatches, UI's and so on. Misc buffs are things like extra damage and class xp for the current week
