ah, you'll need a modloader first, curseforge is a pretty good one. After that download the mod into a folder that is created by the app (no need for file explorer) then after its downlaoded hit the play button under the folder.

you don't, nothing will work with it, there are better alternatives and it barely boosts anything in modded

for Forge, use Embeddium to boost the performance, Oculus to use Shaders (requires Embeddium), Fusion (Connected Textures) for connected textures resource packs, Forge CIT for name dependent textures
for Fabric and NeoForge, use Sodium to boost the performance, Iris to use Shaders (requires Sodium), Continuity for connected textures resource packs, CIT resenew for name dependent textures
for all, Entity Model Features and Entity Texture Features for resource packs with custom entity models and texures, LambDynamicLight for Dynamic Light
For older versions there's Angelica for 1.7.10 which also has shader support, and for 1.12.2 there's Relictium, a port of Sodium
to summarize, Optifine is never worth using anymore
