Could be that he wanted fire wands broken, and doing max ones gives an error?

I got the same errors as OP, but for shields and stone wands. They transmuted but didn't count to quest progression. I was doing two at a time, not sure if that has any impact

Ok, I am unable to progress with shields regardless of how many I mutate at once

Quitting the game seems to have done the trick. Maybe an error related to keeping the game open when the quest becomes active
