Seed of life did not trigger when I had it as 2nd equipment (comes with the new boss item). The second equipment can be triggered by pressing E+Q, but I am not sure if I should press it when I died earlier.

passive equipments arent considered when in your off slot on mul-t so this likely is the same

actually, I got a strange interaction, where I had the seed of life as a second equip, and the scanner as the first. I died, got back by the seed, but instead of chaging the seed appearance, it "consumed" the scanner and got a second seed.
I couldn't use the seed as it was changed(that you plant on the ground and heals you), and then i died for test, and actually ended my run, without triggering the seed revive

I had a similar thing happen when I was holding tonic and tricorn (with gestures), it auto-used the tricorn but turned my tonic into a used tricorn somehow.
Thankfully I was able to get it to turn back into the tonic by swapping enough times (both between held equipment and with an equipment on the ground), but I would have lost that run if I hadn't been able to.
