You have to die by ignite debuff, it may happen that you get the glitch where it doesn't save your progress once you leave the game, in order to prevent it everytime that you finish your run go to the main menu in game and move around in the menus this force the game to save your progress.

I have done that to an extent to let the game recheck my progress for things and some of the achievements I don’t have yet update their progress bars to show me how much more I have to do. I tried for like 6-9 straight runs of dying like that and it won’t count. I’m gonna try again when I get time

Don't activate glass artifact because you may die by the hit of the enemy and not by the ignite debuff.
If you activate glass artifact i would recommend you to grab so you can survive the hit of the enemy and the ignite debuff kill you.

You have to stay under 50% max health while charging the teleport it doesn't matter if it's 1000 health or 10 you have to stay below 50% all the time.
In short glass artifact makes it harder.

Simply manage your health via your abilities,
The third hit of the primary heals 60% of the damage deal.
The robot abilities consume % current health so you can maintain low health using them.
It is easier to do on stage 1 or 2, after that it becomes much harder to do because you will have some items that may molest you and the enemies starts to become stronger than you.
You can do it on

Yep, because of two things,
1- tier 2 elites (malachite and celestial) can spawn after stage 5 or stage 6, so when you teleport to commencement first loop it's stage 6 so the tier 2 elites can spawn.
2-welcome to console land buddy where the lunar chimeras on commencement can spawn as any elite where they supposed to only spawn as perfected elites, it is a bug.
I am not even mad, i was feeling tired and i didn't realize that i was inside of the glacial elite death explosion, after that i laugh a little because this came up to my mind.
