Seems easy to reproduce, i get it every time for now.

Seems like that Vullfy might have Burglar for it's trait, and it's not allowing Reactive Vial to trigger because Rhoulder gets exhausted by plague.

The "burglar" trait should have been print on screen then. And i got the same problem with other attacking tem like mushook uppercut and garyo water attacks

Did you recent change any of the tems on your pro team without moving the items? I had the issue that the items didn't properly update and it wasn't registering reactive vial amd scope properly
If you try re-adding the gear on your prosquad it should fix it
