1 - The hat should come lower, nobody wears a hat at the tip of their head
2 - The shirt is too plain, add some detail
3 - The mask is too tiny, maybe instead of straps you should just make the mask cover his full eye area

The doll's body looks like a dome. She needs to be much slimmer and her torso to head proportions aren't right.
The dividing balloon line (string) doesn't look right. Its color isn't good either. Its curves were hand-drawn as well.
Is the blood part of the eye item?

So suggestions:

Change the doll's clothes from looking the same as the avatar. If it's a color that gives contrast, would be nice.
Lower the doll and make her slimmer, or give signs to the start of a lower body, but leaving her with a torso so long for her head isn't right.
Make sure the string color isn't pale, and that any curves aren't drawn by hand. Try black or very dark blue (almost black)
