The matter wave projector is unstable. Even pre-apocalypse, when Insight were experimenting with it, they lost an entire wing of their lab and countless staff to "manfunctions" with the device. Over the years and as it is used, these glitches get worse, to the point where, in r6a1, it randomly warps us just from touching it. R8 is a spaghettified mess, a "matter wave collapse" of roughly a dozen different universes (worldlines) being slammed together. The code got too corrupt, so to say.
