So you have a 1/136 chance to roll the table. If you hit it, you roll on the unique table like the normal table, but just locked to the uniques. Each unique has the listed rarity

The vestige needs 3 rolls to be dropped. The first 2 are turned into normal loot and give an invisible check on your account, the 3rd will drop the vestige and reset the check counter

There is no way to know when you hit the invisible roll on the vestige
