Me and another user have attempted to upload a new version of the image file https://rainworld.miraheze.org/wiki/File:Subterranean_Map_(Vanilla).png - however, it appears that the image will not update to match on either the thumbnail or the full image. The intended latest version is the image linked on this post. I believe this is likely a cache issue, as clicking the Purge button results in the error "Purge failed".

Try use it this way:
add ?action=purge behind the websites url
wiki/pagename?action=purge
replace pagename with the page you are on

Sometimes thumbnails need some time to update. Clearing your browser's image cache can help
it's not the thumbnail, it's the file itself
Oh? If you view the source image it should be the correct one
using ?action=purge on the source image changes it to the correct one but on reload it'll swap back to the old version

i uploaded and reverted an optimized version of the file in hopes that it would fix it
It's possible you didn't revert to the right version. Can you try again?
it seems like the cache fails to update for the current revision but it'll work perfectly fine for all previous revisions
i did revert to the correct version but i can do so again
With such things it doesn't hurt to double check
(actually i can't because the files are identical)

Hmm. If it prevents you with a warning they're the same, then please give it some time. As far as I'm aware there are some cache issues currently overall
