I would double check that you had met all the requirements, but there is also a bug atm where if your game crashes (not server crash) that you can't get prime elder. Did you get a 'fatal error' popup at any point? that would do it.

Or, if you have entombed and got respawned as a dino with a different gender + a skin, that dino will not be able to get prime.

Wow i've really got no idea why that could happen. My guess is that maybe during a server crash it forgot some data like how long you've been growing for, so it might have forgotten that you went to a sanc and hence you didn't get prime. Normally that only happens when you get a 'fatal error' notification though. This might warrant a bug report if i'm not forgetting something!
