This happens rarely, maybe once every 2 weeks, but it would be nice if it never happened, does anyone know why this could be happening?

Next time this happens, press Win + Shift + Ctrl + B see if the system recovers from that. If that works, you most likely have something wrong with the display drivers and you would have to run a DDU and do a fresh install of the latest drivers.
If it doesn't recover with the keyboard shortcut, it's most likely an issue with some other driver or there's an issue with the OS.
