Depends on the overall airflow setup and where other components are positioned around said cooler.
And even then, it takes quite the amount of ambient heat in the case forced through the same air cooler and out somehow in order to make that happen. Because of that, your temps being worse on the cpu from using an exhaust setup is either unlikely or exaggerated.
You'd have to purposefully do stuff in the case and with the components, as well as using a less than adequate cooler in order to make that even a thing that in most cases isn't really that possible because there isn't one designed so horribly or just... Traps all the heat with only way out.

the simple answer is "depends", but generally you want airflow to go front to back, bottom to top

That is the point though that I'm making. In most cases, it's fine as long as the rest of the airflow setup makes sense or at the very least exists in the system. I'm basically saying you'd need to complicate things intentionally to get actually worse results. Otherwise, you temps will normally be fine doing cpu-fan exhaust.
