Wanted to say this earlier, but my phone was losing connection like a bastard. I'm gonna just touch on the madison thing here. I work in a large company (Won't say who for obvious reasons) and we have a clear HR policy that deals specifically with those issues that were raised. I imagine LMG has a very similar policy that they lay out during the interview and onboarding process. It is possible she just forgot about it and didn't inquire about it later down the road. It's also possible she did know and felt unsafe reporting it due to sensing that there would be retaliation. Or, it's possible she did know, refused to follow policy and kept it under her hat to speak about it after her tenure was finished to avoid real or imagined retaliation.
