Computers generally don't have true random numbers. You have algorithms that given some initial state can be stepped forward to generate a sequence of numbers that are more or less statistically well distributed and "random".
These are PRNGs.

If you put in the same initial state, or seed, you get the same sequence of numbers.
If you generate "enough" numbers with the same generator, it ends up looping around. Some do it very quickly, some do it with unfathomable cycles.

If you're dropping an item in a Diablo-like, you don't necessarily need to figure out the whole item as you drop it.

All you need is to generate a random "seed" number, and store that with the unidentified item.

That number can in turn be used to "seed" a new RNG, which is used to make the choices for that particular item.

The initial seeds you describe are used in the deterministic generation of the initial world state, deciding several factors that make up the world you're inhabiting.

If both sides are supposed to do something "random" and they are not out of sync in how they use the RNG, they magically get the same results from the RNG.

they are not that expensive, but not something you need straight away either
