You can
rare items have max of 6 affixes (3 prefix, 3 suffix)

So you could start with an item with 3 affixes (or even 2) and just slam exalt orbs on it if you really wanted
but that's inefficient really
you want to start with an item that already has stats you want

and there are items in later game that guarantee the exalt slam to give something like
"gives cold affix" but that cold affix can be cold res or cold dmg and it doesn't specify what tier it will be

Ayy nub, i felt that 2h dw on iktab and ekbab was the biggest issue. Due to the hitbox shenenigans and pushing around, the 2nd mace hit miss like 70% of the time. After switching to 2h mace and a shield, it hits more consistently.

yeah I know you're an artist, so aesthetic aspect is ways more important than practical aspect, but when you reach endgame, by refusing to use a loot filter, you accept that you will either play much slower than other people so that you can collect loots carefully, or you play the same speed and miss out on lots of divine and other good currencies

and there will be a point where loots fill the whole screen, maybe t10 or above, and that's where it's impossible to spot the good loots by naked eye anymore
