This should only happen when you originally create the note with a cloze 2 (c2) and then edit it to remove the c2.

This is easy to do accidentally by using the wrong shortcut when clozing text.

Anki doesn't delete cards automatically, even if they'd display nothing, so it asks you to delete them manually.

Does it really? I think it's more likely that you're mis-seeing or misunderstanding something.

Can you search in Browse for "the ciliary muscle relaxes the suspensory" (specific text from the card) to show which cards come up?

I see, thanks

i always delete cards with this button as you can see in the dropdown menu, but when i use it in this case it deletes both

No, it's the same note. But if you preview each card ctrl + shift + p you'll see that it's two different cards: an empty one and one with the c1 cloze.

That option deletes the note, not the card.

The note is the collection of information. A card is the way that information becomes a question and answer pair. You can have multiple cards for one note.

Normally, there is no way to delete a single card from a note. The only way to do it is to use the Empty Cards tool, which will remove cards that don't have anything on their front.

Go to Tools -> Empty Cards to remove cards that show the error you took a screenshot of above.
