I put all my decks in an All deck bc I want to review everything randomly shuffled together. However, my notes could be a single character (F/Foxtrot) and sometimes I don’t remember the context I made the card in. Is it better to modify the card template or front, or is there a way to see where cards come from when reviewing on iOS?

I think what you're asking for can be solved with {{Deck}}/{{Subdeck}} Special Fields on your template -- https://docs.ankiweb.net/templates/fields.html#special-fields. But let me know if I've misunderstood.
Sounds about right. Is that the way most people would approach this? Modifying all templates?
It's one way! If you have tons of different note types for different subdecks/subjects, there might be other ways to differentiate them. But if you use the same note type for pretty much everything, this is a fast way. You might want to format the deck/subdeck name to be small and unobtrusive on your cards -- enough of an indicator, but not distracting.
