On my rp server experience, our wars were planned. OCs weren't automatically able to make moves. We had to word it as attempted/tried etc and the opponent had a chance to dodge or take the hit. This usually played out using strengths and weaknesses in mind and OCs weren't allowed to dodge every attack. every 1/3 times or 1/5 times the OC had to take a hit. But this was with individual characters and not a whole country.

I think we also once had a system where we rolled a dice. 5-6 was strong attack. 3-4 was weak attack and 1-2 was a miss.
