Most, if not all, prismatic unlocks are gonna come from chests in various activities. Some will appear in open world, others from adventures, and the rest come from the campaign.

The only part of Prismatic that is guaranteed to not come from a chest would be the new super related to your current class.

Everything else is hidden within the game.

There is a hidden chest within the Iconoclasm campaign mission, in one of the areas right before Witness fight.
