Not always. Usually there is a mix of the 2: People around your skill level that are below 100 ping etc.

Some games do an "unranked" mode of some sort that prioritizes connection only. Street Fighter 6 Casual Mode is like this (though I think they changed that recently), where was Ranked mode is connection AND skill/rank based.
