Umbreon is correct, the 3rd isnt meant to be in the game and theyre working to program it out

the fourth doesnt happen at all, never will, and is disturbing to suggest as something to add in as an option

Incest, as Jir mentioned, is something the team want to try and patch out at some point but it will take time to implement a family tree, so this is not something that will be added intentionally in future.

One thing I will say is yes, Cult of the Lamb has some dark themes. But it's not meant to be a real life depiction of Cults where things like this can happen. Ultimately it is a teen game and there are lines we don't want to cross.
