The gender ambigiouity is one of my favorite aspects of the game to be completely honest
Some of the cultists might also not be tied to just one particular gender, if that's what this suggestion is referring to

you can just headcanon the lamb or followers as whatever gender you want them to be, the game doesn't need to give an exact correct answer to what their gender is
Do not add genders I hate dealing with pronouns
Honestly, I love how the only canon gendered terms are the pronouns of non-follower npcs, excluding the obvious npcs such as Shamura, and the that's it. The wedding dress and suit aren't for any specific gender either ||because there isn't one, but also because it isn't for a specific gender anyways-|| and all followers are "nude" the same way and like I said: I love that aspect of the game
Followers are like minecraft cows, they are genderless and spawn babies through magic

theres no reason for there to be gender
they could have it be like in halo were there was skulls you can turn on to ramp up the difficulty even more they could have that and call it like sins or something idk

There are harder and survival-related modes already though? You said that you didn't play the game yet, so give it a go with those settings then feel free to make a new suggestion (separate from this one that's specific to gender) if harder is still wanted.
